In order to expedite your medical record request, HIPAA requires Mountain Heart to receive a completed release of information before sending out your records. This ensures that your health information is protected. You can also access your medical records through our patient portal (currently under construction).

Call 928-226-6400 and our front office team can direct you to our prescription refill line to expedite your medication refill request. Our Prescription mailbox is checked every two hours, at 8am, 10am, 12pm, 2pm, and 4:30 pm. Our goal is to process all refill requests within 48 hours. Please be prepared to provide the patient Name, telephone number, date of birth, medication, dosage, preferred pharmacy, and to allow for 2 business days for prescriptions to be processed.
